The growth of evidence-based medicine has occurred against a backdrop of health care reform, managed care, cost containment and quality improvement, and clinicians have been urged to adopt the rigours of science while remaining true to their 'clinical judgement'. This tension - between efforts to make medical practice more scientific and the suspicions of many clinicians - has caused one of the greatest practical and ethical challenges in the history of the health professions. This incisive book reviews the history and conceptual origins of evidence-based practice, and discusses key ethical issues that arise in clinical practice, public health and health policy. Essential reading for all physicians, and practitioners in epidemiology and public health, it will also be suitable as a text in medical and public health school courses on evidence-based practice.

The concept of community is increasingly the focus of political argument in Britain, the United States and elsewhere around the world. The sense people have of belonging to coummunities provides a powerful motivation which continues to affecct the political and social face of the world. Recently, debate about the relationship between individuals and their communities has become central to the making of both, American and European social policy. In the United Kingdom this is especially apparent in the area of health care, where ideas of community have informed recent legislation concerning com

"Perhaps no medical breakthrough in the twentieth century is more spectacular, more hope-giving, or more fraught with ethical questions than organ transplantation. Each year some 25,000 Americans are pulled back from the brink of death by receiving vital new organs. Another 5,000 die while waiting for them. And what distinguishes these two groups has become the source of one of our thorniest ethical questions." "In Raising the Dead, Ronald Munson offers a vivid, often wrenchingly dramatic account of how transplants are performed, how we decide who receives them, and how we engage the entire range of tough issues that arise because of them. Each chapter begins with a detailed account of a specific case - Mickey Mantle's controversial liver transplant for example - followed by careful analysis of its surrounding ethical questions (the charges that Mantle received special treatment because he was a celebrity, the large problems involving how organs are allocated, and whether alcoholics should have an equal claim on donor livers). In approaching transplant ethics through specific cases, Munson reminds us of the complex personal and emotional dimension that underlies such issues. The book also ranges beyond our present capabilities to explore the future possibilities in xenotransplantation (transplanting animal organs into humans) and stem cell technology that would allow doctors to grow new organs from the patient's own cells." "Based on extensive scientific research, but written with a novelist's eye for the human condition, Raising the Dead shows readers the reality of organ transplantation now, the possibility of what it may become, and how we might respond to the ethical challenges it forces us to confront."--Jacket.

Bioethics tackles the most vexing problems in health care and the biological sciences: from reproductive technologies to euthanasia, the AIDS epidemic, mapping the human genome, human subjects research, and health care reform. Yet surprisingly little attention has been paid to the special problems faced by women and to feminist analyses of current health care issues other than reproduction. This breakthrough volume of original essays authored by leading figures in bioethics and feminist theory moves beyond the areas of reproduction and nursing, taking bioethics into new territory. The book starts with an investigation of the relationship between feminism and bioethics and introduces different approaches to the problem. These chapters stress the importance of liberal feminism that prefers feminist over feminine analysis, integrates the experience of women of color, draws from the women's self-help movement, and uses the feminist stand-point theory. In the second part of the book, the authors apply the feminist perspective to different bioethics problems: euthanasia, AIDS, the definition of health, doctor-patient communication, the Human Genome Project, the conduct of biomedical research, and health care reform. They demonstrate the gain and benefit that results when bioethics pays attention to gender and feminism. This volume will change the way bioethicists, students, patients, and the public think about these profoundly challenging problems.

Whether the law should permit voluntary euthanasia or physician-assisted suicide is one of the most vital questions facing all modern societies. Internationally, the main obstacle to legalisation has proved to be the objection that, even if they were morally acceptable in certain 'hard cases', voluntary euthanasia and physician-assisted suicide could not be effectively controlled; society would slide down a 'slippery slope' to the killing of patients who did not make a free and informed request, or for whom palliative care would have offered an alternative. How cogent is this objection? This book provides the general reader (who need have no expertise in philosophy, law or medicine) with a lucid introduction to this central question in the debate, not least by reviewing the Dutch euthanasia experience. It will interest all in any country whether currently for or against legalisation, who wish to ensure that their opinions are better informed.
